As the U.S. population ages, communities need new ways to make high-quality, age-friendly care accessible to older adults. In NEJM Catalyst, researchers from the University of Tennessee at Chattanooga and Mobile Health Map share early findings from MobileMOC, an interprofessional mobile health model bringing primary and preventive care to rural communities. Centered on the Age-Friendly Health Systems 4Ms framework, the model integrates mobile clinic visits, senior center activities, and interprofessional education to support older adults’ health and well-being. The publication explores lessons from implementation and challenges in measuring impact and sustaining this approach.
